Report a Change

Hancock Metropolitan Housing Authority (HMHA) requires all program participants to report changes in income or household composition in writing within 10 days of the change. To report a change, please complete the Household Update Change form which includes:

Household Update Change Form - This form must be completed in its entirety, signed and dated. All members of the household 18 years of age and over must also sign the change form. Incomplete forms may delay the processing of your information.

HUD Form 9886 Authorization for Release of Information - This form requires the signature of every member of the household who is 18 years of age and over. It also requires the Social Security number of the head of household.

HMHA Consent Form - This form requires the signature of every member of the household who is 18 years of age and over.

In addition to these forms when you submit your change, please enclose the following documents:

LOSS OF INCOME - Verification from the source of lost income. For example, termination notice from employer, print out from child support, etc. If this change will change again within 30 days, HMHA will not process a change.

INCREASE IN INCOME - Verification from the source of income. For example, pay stubs

ADDING SOMEONE TO THE HOUSEHOLD - Verification of birth and Social Security card. Unless it is a newborn child, HMHA will require custody documents.

ADDING SOMEONE OVER 18 TO THE HOUSEHOLD - The additional person must provide a photo ID, proof of birth, Social Security card, verification of income, and written approval from the landlord to add the person to the lease agreement. HMHA must complete a criminal verification and has the right to refuse to add anyone to the household. HMHA will notify you if the addition is approved and will process an interim change form.

HMHA is required by HUD to complete third party verifications on all changes reported. Please refer to the chart below for the effective date of your change. This will apply to most changes except in cases where the income change will last less than 30 days, where there is a sanctioned welfare benefit, or a recent move.

When will change be effective? 

Paperwork Sumitted by Tenant - Decrease in Income

Before the 15th of the month -  Decrease 1st of the following month

Between the 15th and last day of the month - Decrease 1st of the following month

Paperwork Sumitted by Tenant - Increase in Income

Between the 1st and last day of the month - Increase 1st of month after 30 days the month following
